What We Do
Licensed by the New York State Department of Health, Concept: CARE works within a standard of operation to maintain the highest level of excellence in caregiving. As experienced health care providers, our staff works in conjunction with family and all members of the health care team to provide assistance in formulating an individualize plan of care for each person we care for. Team members are either licensed, certified or degreed and are comprised of Registered Nurses, Home Health Aides, Personal Care Aides, and Companions. We offer hourly, Live-In, 24 hours round the clock care services, assistance with staffing needs while hospitalized, and companionship services for medical appointments and special events. Your care is our priority. If we don’t know the answer to a problem we will assist you in finding it.
